Fatigue identified in fishing vessel sinking

The State's investigation into the sinking of the fishing vessel Tit Bonhomme last year with the loss of five lives has found crew fatigue to have been the "single overriding causal factor". The Marine Casualty Investigation Board (MCIB) report has been unable to establish who was on watch when the vessel ran aground on Adam's island at the mouth of Glandore harbor in the early hours of January 15th, 2012. However, examination of the hull by Naval Service and Garda diving teams established that the 21-meter vessel was on autopilot and the engine in gear with a course set for Adam's island, and there was no "off course" alarm system. Tanker ran aground in Svir River

   The inland tanker Volgoneft-162 (IMO: 8867131) ran aground in the Svir River, north-east of St. Petersburg, Russia. The tanker was making a voyage between Kashper and St. Petersburg, carrying 4433 tons of heavy oil. Damage into the propulsion system and the steering gear changed the course and the ship has diverged from the navigation channel and ran aground into the mud bank. The tanker Volgoneft-162 got damage in the bow, near the bow thruster. There is no information for injuries and pollution of the water after the accident. On the position there are coast guard tankers and rescue boats.
